A New Patient/Resource Request Screening Form is the first step to getting started in obtaining services or resource suggestions from our center. The screen gives us a starting snapshot of your situation and needs so that we can most efficiently be of assistance. Screens are reviewed by our team and an intake coordinator will reach out within 1-2 business days if we need additional information, have resource options we believe may be helpful to you, and to provide a point of contact before a team member reaches out to schedule.
